de la place des associations triples fixes réservée aux patients atteints de BPCO sévère traités de façon non satisfaisante par une bithérapie associant un CSI à un LABA ou un LABA à un LAMA;
de la possibilité de prescription des CSI, LABA et LAMA par tout médecin sous forme d’associations libres, et du risque de prescriptions dissociées non adaptées ;
de l’existence de multiples dispositifs d’inhalation, et du risque de mauvaise technique d’utilisation pour chaque dispositif ;
de la mauvaise observance aux divers produits inhalés ;
du mésusage élevé observé avec les associations triples dans le traitement de la BPCO ;
du mésusage du même ordre lors de l’instauration de traitement par les médecins pneumologues et par celui des médecins généralistes ;
la Commission estime que :
la prescription initiale de TRELEGY ELLIPTA (fluticasone / uméclidinium / vilantérol) peut être instaurée par tout médecin sous réserve de la réalisation d’une exploration fonctionnelle respiratoire (EFR) confirmant l’existence d’un trouble ventilatoire obstructif, dans l’année précédant l’instauration du traitement par cette spécialité ;
un suivi par un pneumologue doit être réalisé dans l’année suivant la prescription initiale de TRELEGY ELLIPTA (fluticasone / uméclidinium / vilantérol) afin de s’assurer de la nécessité d’une corticothérapie inhalée.

the role of triple fixed-dose combinations reserved for patients with severe COPD, who are not adequately treated by an ICS and LABA combination or a LABA and LAMA combination;
the possibility for any physician to prescribe ICS, LABA and LAMA in the form of free combinations, and the risk of inappropriate separate prescriptions;
the existence of multiple inhalation devices, and the risk of an incorrect technique for use for each device;
the poor compliance with various inhaled products;
the high level of misuse observed with triple combinations in the treatment of COPD;
the similar levels of misuse observed following the initiation of treatment by pulmonologists and general practitioners;
the Committee deems that:
the initial prescription of TRELEGY ELLIPTA (fluticasone / umeclidinium / vilanterol) may be initiated by any physician provided that lung function tests confirming the existence of obstructive airway disease have been performed in the year preceding the initiation of treatment with this medicinal product;
follow-up by a pulmonologist should be performed during the year following initial prescription of TRELEGY ELLIPTA (fluticasone / umeclidinium / vilanterol) in order to ensure that inhaled corticosteroid therapy is necessary.
